Ways to Access Your Medical Records

Find On MyChart

You can get a digital copy of your medical records on MyChart, free of charge. 

  1. Click on “Your Menu” and select “Obtain or Share Your Record.” 
  2. Choose who you want to share your medical records with. 
  3. Click on “Request a Formal Copy” 
  4. Complete the form and click “Send Request.” 

Once approved, you can view your medical records in the “Documents” section of MyChart.

Contact Your Doctors' Office

You can contact your doctors' office directly to get access to your medical records.

Request Paper Copies

There is a cost for retrieving medical records, unless they are being sent directly to a health care facility for continuity of care.

The information can only be released with authorization from the patient or their legal guardian. The law requires that the authorization include the patient’s signature and the date the request is being authorized. The law requires that medical records be kept for 10 years after the patient reaches majority.
